Japan's population structure is aging at a rate unprecedented in other countries, and the nursing care field is chronically understaffed. In the future, it will be essential to improve operational efficiency and productivity at nursing care facilities through the collaboration of medical and nursing care information among multiple professions, DXing of nursing care records, and the use of IoT and nursing care robots. In addition, there is a need to improve the environment of the medical system for emergency care for those requiring nursing care, and to realize prompt evacuation support in the event of a disaster.
Overview
By DX of the cooperation of medical and nursing care information among multiple professions, we will strengthen the community's monitoring system and build a service provision system that allows the elderly to live with peace of mind. In addition, as emergency medical countermeasures and disaster countermeasures for persons requiring evacuation action, routinely accumulated medical and nursing care information, ACP, and DNAR information can be used for prompt life-saving activities.
Community Integrated Care
Team × MySOS × IoT Collaboration
In addition to records created by caregivers and nurses, personal health records registered in MySOS by residents and vital information obtained from IoT devices are consolidated in Team, providing a forum for real-time information sharing among multiple professions.
It also facilitates patient referrals between medical institutions, coordination with caregivers at the time of discharge, and post-discharge follow-up.
Medical Information (EHR) System Linkage
Information can be linked in both directions with medical information systems using IHE-ITI.
By referring to the medical information system's drug information and test results with Team, it is possible to quickly grasp the situation when assisting with medication during nursing care services or during emergency transportation.
Emergency Medical Countermeasures
We will share medical and nursing care information of local residents from normal times, and establish an environment that allows cooperation with firefighting and logistics support hospitals.
With the increasing number of emergencies for the elderly, we will be able to provide appropriate treatment and transport according to the severity of the illness and the person's wishes at the time of emergency transport, and maintain an acceptance system for seriously injured patients and patients requiring highly specialized treatment at regional flagship hospitals such as advanced life support centers.
Disaster Countermeasures
By using a system that holds the latest family, housing, nursing, and care information for persons requiring care, etc., as a multidisciplinary collaboration system, the system reduces the workload of care managers and others in creating and managing individual evacuation plans, and provides prompt evacuation and rescue support in the event of a disaster.
